Patient's FAQs

What is compounding?
Compounding is the art and science of combining, mixing, or altering ingredients to create a medication, prescribed by a physician, tailored to an individual patient’s need.  Compounded medications are not commercially available in the formulation or dosage form dispensed. Hotze Pharmacy uses only USP/NF grade bulk products to compound your prescriptions. Our preparations are lactose free, preservative free, and compounded with a methylcellulose product which offers slow release properties.

How do I get my refills?
When you are in need of refills, you can call us at (281) 828-9088 or toll-free at (877) 640-5248, or you can submit your request online at www.hotzepharmacy.com/refills.cfm. If possible, please have your prescription numbers ready.  Prescription processing may take up to 24 hours. Additionally, if you would prefer to have your order shipped, please allow 2 - 5 business days for delivery.

What is the cost of shipping?
We can ship the prescriptions via UPS at no charge to you for shipments over $75, and for a $7 charge for shipments less than $75. Upon request, expedited shipping is available for an additional fee.

Will insurance pay for my prescriptions?
We can not guarantee that the cost of these medications will be covered by your insurance provider.  However, with each prescription filled, we will provide a Universal Claim Form for a Compounded Medication, which contains the most commonly requested information. The box at the bottom half of the page entitled ‘Ingredients’ contains the ingredients, quantities, and NDC numbers.  Only the NDC number for the active ingredient is included, because that is the only ingredient for which the insurance companies will reimburse; the inactive ingredients are not relevant to the claim.  Because our formulas are proprietary, we are unable to disclose information about the inactive ingredients.

Please note that the Universal Claim Form is provided for compounded prescriptions only. The NDC number for commercially available drugs (ie Armour Thyroid, Diflucan, Cyanocobalamin, Depo-Testosterone) can be found at the bottom of the Drug Information Sheet on the perforated receipts.

If you have further questions regarding your coverage, submitting this claim form, or receiving payment from your insurance company, please contact your employer benefits manager.

How can I save on the cost of my prescriptions?
Our pricing is a reflection of our commitment to consistently provide the finest quality compounded prescriptions available. It reflects not only the labor involved in preparing the hormone prescriptions, but also the extensive quality control testing which we employ to ensure that our compounds are of the highest quality. Additionally, we make a concerted effort to be available for any counseling or questions you may have.

With all of that said, there are ways that you can save on the cost of your prescriptions:

  1. A 10% concession is applied to all orders placed online at www.hotzepharmacy.com/refills.cfm.
  2. The pricing for our compounds is based on labor, not on the strength, so you can save money by combining multiple capsules into 1 higher strength capsule.  For example, if you are taking 2-12.5 mg capsules of progesterone, we can request to increase the strength, so you take 1-25 mg capsule.
  3. We offer concessions for multiple month supplies of compounded prescriptions; 5% for 2 month and 10% for 3 month. (Note: This does not apply to manufactured drugs, such as Armour Thyroid, Depo-Testosterone, or Diflucan.)
  4. By signing up for our Premier Program, where your prescriptions will be sent to you automatically every 1, 2, or 3 months, you can save 5% per order.  You will want to make sure that you and the doctor have determined your ideal doses before signing up, as there are often dosage adjustments during the first few months.
